Study: Incorrect medication information often entered into medical records

A new study published in the Journal of General Internal Medicine shows that medication errors are entered into patient's medical records at a rate of 1.5 times per hospitalization, reports The Boston Globe. Researchers from both Brigham and Women's Hospital and Massachusetts General Hospital in Boston audited the medical records of 180 patients over the span of two months to measure how accurate the medications listed in their records were.

Most problems arose from the medication list gathered upon a patient's entry into the hospital. Of the wrong information recorded, 75% could have potentially harmed patients once they'd been discharged.
