PA Patient Safety Authority releases annual report

Pennsylvania hospitals are utilizing advisories and reporting more errors and near misses to the state's Patient Safety Authority. Reports are up in the state over this time last year, according to an authority press release.

Near miss reports increased over the last year as well, while reports of errors that led to injuries are down.

A recent survey by the authority found that hospitals have made more than 500 changes due in part to the numerous advisories issued over the past year.
