Medical errors go unreported at NJ, PA hospitals

Despite state laws requiring hospitals to report medical errors to state agencies, hospitals in both New Jersey and Pennsylvania have failed to report serious medical mistakes in the past year, The Philadelphia Inquirer reports.
Such incidents included objects left behind after surgery and excessive postoperative bleeding.
Twenty six states require hospitals to report medical errors, and within the next year, three states (Massachusetts, California, and Minnesota) will also publicly report this information.
