Medical error leads to discovery of protocol violations

Federal inspectors found several pharmacy protocol violations in the wake of a patient's death at Sierra Vista Regional Medical Center, according to a San Luis Obispo (CA) Tribune article. The hospital said it has come forward with the information as part of its commitment to transparency and patient safety.

Hospital officials said they have addressed the issues raised, which include pharmacy technicians performing tasks that the state required be completed by pharmacists, failure to track drugs dispensed by Pyxis machines, medication counting and tracking violations, and incomplete competency training in a number of areas.
