Indiana releases preliminary error report

The Indiana Department of Public Health has released its preliminary 2006 report on medical errors. The findings show there were 77 reported adverse events in the state last year.

Of those 77 errors, 23 were incidences of phase III or IV pressure ulcers. There were 21 reported cases of a foreign object left in a patient. There were nine cases of wrong site surgery.

The state based its error reporting system on the National Quality Forum's list of 27 "never events." There are 287 facilities required to report to the state. Of the 77 errors reported, 72 came from hospitals and five came from ambulatory facilities.
