When discussing patient safety, we often talk about how systematic processes can be fixed. We discuss putting in place methods to prevent patient falls and infections. Overlooked sometimes can be the misdiagnoses. Misdiagnoses can be the result of an incompetent physician, but more likely is that a physician is rushed or worried about practicing too much "defensive medicine," ordering every test possible to ensure legal troubles don’t haunt them later.
