Patient Safety Monitor Alert

April 18, 2007

The House Committee on Energy and Commerce has asked AHRQ to study the effect of worker fatigue on the rate of medication errors, according to a report by AHA News Now. The letter, sent to AHRQ by the committee, outlines concerns that date back to the 1999 Institute of Medicine (IOM) report, To Err is Human.

The letters suggested the IOM conduct and publish the study.
